Avherald riporta questi due inconvenienti accaduti oggi in Australia.
Incident: Virgin Australia B738 at Mildura on Jun 18th 2013, landed below weather minima and substantially below final fuel reserve

Per fortuna entrambi si sono conclusi senza incidente.

E' andata bene, i piloti son stati professionali (immagino il livello di stress cui eran sottoposti nel momento in cui "hanno raddrizzato gli schienali nel cockpit per cercare di massimizzare la loro possibilità di intravedere qualcosa nella nebbia"),
mi vengono in mente due o tre compagnie che quanto a policy relativa al calcolo del fuel di riserva... meglio che sto zitto.

E se l'aeroporto fosse stato chiuso?A Qantas Boeing 737-800, registration VH-VYK performing flight QF-735 from Sydney,NS to Adelaide,SA (Australia) with 146 passengers and 6 crew, had been dispatched with a weather forecast, that indicated clear weather in Adelaide with no necessity to plan for an alternate aerodrome, the crew departed with sufficient fuel to arrive at Adelaide with 2800 kg of fuel remaining giving them about 45 minutes of flying time on top of the required final reserve. Si, è possibile partire senza aver un alternato, rispettando certe condizioni (EU OPS: previsione meteo +/- 1h sora un certo limite e 2 piste separate, oppure aeroporto isolato). (c) An operator must select at least one destination alternate for each IFR flight unless:

Anche in US la regolamentazione per gli alternati è simile.
Domestic flight : indicated wx at destination arpt. @ ETA +/- 1 hr. must be V = 3 SM (or greater) and C = 2000' (or greater).
Invece l'alternato è indispensabile quando: indicated wx @ dest. @ ETA +/- 1 hr. is V < 3SM or C < 2000'.
Per Flag operation invece è sempre necessario un alternato se l'ETE > 6 hr. Per quanto riguarda ETE <6 hr. vi risparmio la regolamentazione che è una pappardella infinita.
Second alternate è necessario invece quando indicated wx @ dest. and 1st alternate is marginal . Non esiste una precisa definizione di "marginal". E' a discrezione del dispatcher o del PIC, all'esame pratico FAA mi hanno chiesto cosa fosse per me marginal , ho risposto "ceiling 100' and visibility 1/2 SM above alternate mins" .

un comandante Alitalia un tempo mi disse che questa procedura di partire senza alternati, anche se legale, era rischiosa e ti rendeva carico di una enorme responsabilità in quanto lasciavi poco margine al "fato" ( leggi di Murphy ).
