Dato che siamo in area tecnica al momento non faccio ipotesi, certo non sarebbe la prima volta che il distacco di una fan blade provoca il cedimento dell'intero inlet.
Molte fonti (tra cui anche AvHerald) riportano le similitudini con l'altro evento del 2018 che ha interessato un 772 (fan blade fracture, sempre con danni ad engine cowl and inlet): potrebbe essere (tra l'altro, stesso motore, PW4077), ma al momento sono solo ipotesi e non si possono dare cause certe.

Dal danno che si vede alla fusoliera pare proprio che le fan blade(s) partite hanno sfondato anche parte di essa

"The initial examination of the airplane indicated most of the damage was confined to the number 2 engine; the airplane sustained minor damage. The examination and documentation of the airplane is ongoing.
The initial examination of the Pratt & Whitney PW4077 engine revealed:
- The inlet and cowling separated from the engine
- Two fan blades were fractured:
+ One fan blade was fractured near the root
+ An adjacent fan blade was fractured about mid-span
+ A portion of one blade was imbedded in the containment ring
+ The remainder of the fan blades exhibited damage to the tips and leading edges

On Feb 21st 2021 United, Japan Airlines and All Nippon Airways grounded their active Boeing 777 aircraft equipped with PW-40xx engines".

FAA preparerà una Emergency Airworthiness Directive (EAD) che richiede ispezioni dei Boeing 777 con determinati motori PW4000.
Boeing oggi ha dichiarato: "Boeing is actively monitoring recent events related to United Airlines Flight 328. While the NTSB investigation is ongoing, we recommended suspending operations of the 69 in-service and 59 in-storage 777s powered by Pratt & Whitney 4000-112 engines until the FAA identifies the appropriate inspection protocol.
Boeing supports the decision yesterday by the Japan Civil Aviation Bureau, and the FAA’s action today to suspend operations of 777 aircraft powered by Pratt & Whitney 4000-112 engines. We are working with these regulators as they take actions while these planes are on the ground and further inspections are conducted by Pratt & Whitney.
Updates will be provided as more information becomes available".

Quindi non e' un uncontained.
Ci sedemmo dalla parte del torto visto che tutti gli altri posti erano occupati.

Cosa confermata anche da NTSB:
"On Feb 22nd 2021 the NTSB reported in a virtual media briefing that the right hand engine suffered the failure of one fan blade at about 12,500 feet MSL, the fractured blade caused an overload to the neighboured blade which fractured about mid span. Following the failure the crew pulled the fire handle and discharged both fire bottles. Fuel was turned off. The NTSB is looking into what caused the fire to continue. There was additional damage to a composite fairing, the damage was not structural. One of the blades was found in the soccer field on the ground. Following landing the crew assessed no emergency evacuation was necessary. Metal fatigue played a role and is being looked at. The engine's maintenance history is being reviewed. Additional debris was found Monday morning (Feb 22nd 2021) in Arvada about 20nm westsouthwest of Denver Airport. First inspections of the engine do not suggest that anything pierced the containment ring and engine case. At this point the NTSB does not consider the engine failure as uncontained, the containment ring did contain the fractured fan blades, the failure thus was contained".

Essendo in area tecnica non voglio assolutamente scrivere fesserie o cose campate per aria! Parlando però del principio di funzionamento di una turbina su un motore termico, mi viene da ipotizzare che, se giustamente è stato interrotto il flusso di carburante, il fuoco ancora presente nel motore, potrebbe essere stato causato dall' autocombustione, cioè per dirla in termini semplici, stava bruciando in camera di combustione, il proprio olio destinato alla lubrificazione! Magari ho detto una fesseria, ma secondo me potrebbe starci tra le "mille" ipotesi possibili. Comunque aspettiamo le indagini di chi di dovere!

D'accordo, ma quel danno sulla fusoliera poco sotto l'attacco dell'ala non appare di aspetto compatibile tanto con una parte della cappottatura aerodinamica, ma con qualcosa di maggiore compattezza e densità... sbaglio?
